Adoption Agreement

Conditions of DOG Adoption

· To pay the specified adoption fee.

· To contact Foster Pet Outreach if for any reason during the life of the animal, you cannot keep this animal.

· This animal is be to current on Rabies vaccination throughout the animal's lifetime.

· This animal will be kept as an inside pet, allowing it access to the outdoors only for reasonable exercise.

· Agree to provide dogs with an identification tag listing new owners name and phone number within one week of adoption.

· If animal is lost, you will contact Foster Pet Outreach immediately; will contact the Animal Shelter in new owners area and complete a lost card within 24 hours; will place an advertisement under the Lost/Found section (at owner's expense) in the local daily newspaper and will post flyers within neighborhood.

· You will Not to use any animal for medical or any other experimental purposes whatsoever.

· That upon actual conveyance of said animal to new owners, new owner hereby releases Foster Pet Outreach, its representatives, and their designees, as well as all previous owners and handlers of said animal from any and all liability incurred with regard to health, condition, behavior and well-being of said animal.

· That in the event there is a breach of this agreement by the new owner, the undersigned Foster Pet Outreach representative, or such other Foster Pet Outreach designee may reclaim said animal immediately.

· That contemporaneously with the executive of this Agreement, the Foster Pet Outreach representative shall convey, and new owner hereby acknowledge receipt of all rules and regulations of the FPO program. It is firmly understood and stipulated by all parties to this Agreement that they will follow all rules and regulations of said program. This agreement is the entire Agreement between the parties, except as designated by reference, and this Agreement shall be interpreted and enforced under the provisions and laws of the State of IL.

· That a printed version of this agreement will be signed by the adopting person and witnessed by a Foster Pet Outreach representative before the adoption will be permitted to take place.
